Five questions, mostly taps, so clients answer them in under a minute. Send them every week on the same evening. Why these five and what to do with the answers is in Check-in questions for personal training clients.

Weekly check-in

  1. How was your energy this week? Slider from 1 (empty) to 10 (full of energy).
  2. How did you sleep? Five faces, from very badly to very well.
  3. Any pain or soreness that lasted more than two days? Yes or no.
  4. Where, and how bad? Open answer. Only ask this after a yes to question 3.
  5. Anything I should know before your next session? Open answer, optional.

Variations

After-workout check-in. For clients who train on their own: send three questions right after each workout instead of weekly.

  1. How hard was today? Slider from 1 to 10.
  2. Anything that hurt or felt wrong? Yes or no, with a follow-up after yes.
  3. Anything else? Open answer, optional.

Before a progress review. Once every six weeks, add two questions to the weekly check-in:

  1. What has gone better than you expected?
  2. What would you like to work on next?

In Trainnode

Create a survey with these questions under Surveys. Use a slider for question 1, an emoji scale for question 2, single choice for question 3 and open text for 4 and 5, and make question 4 appear only after Yes. Assign it as a weekly recurring survey with a reminder. Set up automatic check-ins walks through every step.
