Intake form and health questions for new PT clients

A free intake form for personal training: health screening questions like a PAR-Q, goals, training history and consent. Copy it and adapt it.

An intake form does two jobs: it tells you whether a new client can safely start training, and it gives you what you need to plan their first weeks. Send it before the first session, so the intake itself is a conversation, not paperwork.

Health

Answer yes or no. If you answer yes to any of these, we'll ask you to check with your doctor before we start training hard.

  1. Has a doctor ever said you have a heart condition, or high blood pressure?
  2. Do you get pain in your chest, at rest or when you're active?
  3. Do you lose your balance because of dizziness, or have you lost consciousness in the past year?
  4. Are you being treated for a chronic condition, such as diabetes, asthma or epilepsy?
  5. Do you take medicines that affect your heart rate or blood pressure?
  6. Do you have a bone, joint or muscle problem that could get worse with exercise?
  7. Are you pregnant, or have you given birth in the past six months?

If yes to any question: what is it, and is there anything we should take into account?

Goals

  1. What do you want to have achieved in three months?
  2. Why is that important to you right now?
  3. What has stopped you before?

Training history

  1. How have you trained in the past year? How many times a week?
  2. Any injuries or operations in the past?
  3. Exercises or movements you love, or want to avoid?
  4. How many times a week can you realistically train with us?

Agreement

I confirm the answers above are correct to the best of my knowledge. I'll tell my coach when my health changes. I've read the [studio name] terms and cancellation policy.

Name, date, signature.

How to use it

  • Read it before the intake, and ask about every yes in person.
  • Store it safely. Health answers are sensitive personal data under the GDPR (AVG in the Netherlands). Keep them where only coaches can see them.
  • Ask again once a year, or when a client comes back after a long break.

In Trainnode, build the form as a survey and turn on Mark as a new-user survey, so it's suggested every time you add a client. The answers are saved on the client's page in your dashboard.
